INSTRUCTIONS:

1) Download the index.php.zip file and unzip it.

2) Open the index.php file in your favorite text editor.

3) Set variable values to suit your needs.

4) Save the file and upload it to the directory of your choice.

5) Be sure the host directory and all subdirectories are chmod 755

6) Be sure all files that you want shown are chmod 644

7) Be sure all directories, subdirectories and files are owned by the
   webserver: (chown -R www:apache directoryname/)
